Methods: A total of 170 AP patients were admitted immediately after the onset of the disease and divided into MAP group (85 cases) and SAP group (85 cases). The levels of serum IL-33, TNF-α, IL-6, and HMGB1, as well as the expression of miRNA-155 in extracellular vesicles (EVs), were dynamically monitored at multiple time points (0 h, 6 h, 12 h, 24 h, 3 d, 5 d, 7 d, 14 d) after admission. Abdominal CT radiomics analyzed the texture characteristics of pancreatic necrosis, and stool samples collected at admission were metagenomic sequencing of the gut microbiome. The Acute Pancreatitis Severity Bedside Index (BISAP) score is calculated within 48 hours of admission. Multivariate regression analysis assessed the independent effects of various factors on the prognosis of mortality groups. Results: Serum IL-33 and TNF-α levels in SAP patients were significantly higher than those in MAP patients (p &amp;lt; 0.05) at all time points, peaked on day 3, and decreased with treatment. The levels of these cytokines in patients with SIRS were also higher than in patients without SIRS (p &amp;lt; 0.05). The serum IL-33, TNF-α levels and BISAP scores in the mortality group were higher than those in the survival group (p &amp;lt; 0.05). Multivariate regression analysis showed that serum IL-33 (OR = 3.21, 95% CI: 1.12–9.23, p = 0.03), TNF-α (OR = 4.05, 95% CI: 1.37–11.96, p = 0.01), and BISAP score (OR = 5.67, 95% CI: 1.83–17.54, p &amp;lt; 0.01) were independent prognostic risk factors. Spearman correlation analysis showed that serum IL-33 and TNF-α levels were positively correlated with BISAP scores(r = 0.68, p &amp;lt; 0.01; r = 0.73, p &amp;lt; 0.01). Conclusion: Dynamic monitoring of serum IL-33 and TNF-α levels combined with BISAP score has important clinical value for early diagnosis, severity assessment, treatment guidance and prognosis evaluation of AP, and provides a basis for accurate diagnosis and treatment.
<keywords/></article-meta></front><body/><back><ref-list><ref id="B1" content-type="article"><label>1</label><element-citation publication-type="journal"><p>Chinese Society of Surgery, Chinese Society of Pancreatic Surgery, 2021, Chinese Guidelines for Pancreatic Surgery, Chinese Journal of Surgery, 59(7): 578–587.</p><pub-id pub-id-type="doi"/></element-citation></ref><ref id="B2" content-type="article"><label>2</label><element-citation publication-type="journal"><p>Wang D, Li L, Wu X, 2022, Advances in Surgical Treatment of Pancreatic Diseases, Chinese Journal of Surgery, 37(6): 456–460.</p><pub-id pub-id-type="doi"/></element-citation></ref><ref id="B3" content-type="article"><label>3</label><element-citation publication-type="journal"><p>Research Progress on the Relationship Between Intestinal Microbiota and Acute Pancreatitis, 2023, China Excavation Journal, 43(5): 321–325.</p><pub-id pub-id-type="doi"/></element-citation></ref><ref id="B4" content-type="article"><label>4</label><element-citation publication-type="journal"><p>Lankisch P, Apt M, Banks P, 2015, Acute Pancreatitis, The Lancet, 386(9988): 85–96.</p><pub-id pub-id-type="doi"/></element-citation></ref><ref id="B5" content-type="article"><label>5</label><element-citation publication-type="journal"><p>Banks P, Boren T, Devinis C, et al., 2012, Classification of Acute Pancreatitis – 2012: Revision of the Atlanta Classification and Definition by International Consensus, Intestine, 62(1): 102–111.</p><pub-id pub-id-type="doi"/></element-citation></ref><ref id="B6" content-type="article"><label>6</label><element-citation publication-type="journal"><p>Bai Y, Shi N, 2015, Clinical Effect of Pantoprazole Combined with Gastric Bismuth Magnesium in the Treatment of Peptic Ulcer, Chinese Medical Review, 12(32): 130–133.</p><pub-id pub-id-type="doi"/></element-citation></ref><ref id="B7" content-type="article"><label>7</label><element-citation publication-type="journal"><p>Sanad M, Challan S, 2017, Radioiodization and Biological Evaluation of Rabeprazole as a Radiotracer for Peptic Ulcer Localization, Radiochemistry, 59(3): 307–312.</p><pub-id pub-id-type="doi"/></element-citation></ref><ref id="B8" content-type="article"><label>8</label><element-citation publication-type="journal"><p>Wang Q, Liu J, Wang D, et al., 2021, Effects of TCM Combined with Conventional Treatment on Intestinal Flora and Inflammatory Factors in Patients with Severe Acute Pancreatitis, China Journal of Emerging Medicine, 30(10): 1862–1866.</p><pub-id pub-id-type="doi"/></element-citation></ref><ref id="B9" content-type="article"><label>9</label><element-citation publication-type="journal"><p>Wang L, 2016, Clinical Efficacy of Aluminum Phosphate Gel in the Treatment of Peptic Ulcer in Children and Its Effect on Disease-Related Serum Indexes, Journal of Practical Clinical Medicine, 20(1): 72–74.</p><pub-id pub-id-type="doi"/></element-citation></ref><ref id="B10" content-type="article"><label>10</label><element-citation publication-type="journal"><p>Dong H, Jin S, Zhu F, et al., 2017, Effects of Aluminum Phosphate Gel Combined with Lansoprazole on HP Clearance and Changes in Serum MMP-9 and GAS Levels in Patients with Hp-Positive Gastric Ulcer, Chinese Journal of Endemic Disease Prevention and Control, 32(9): 1057–1057.</p><pub-id pub-id-type="doi"/></element-citation></ref><ref id="B11" content-type="article"><label>11</label><element-citation publication-type="journal"><p>Zhang H, Li Y, Wang D, et al., 2016, Advances in Treatment of Acute Pancreatitis, World Journal of Gastroenterology, 22(34): 7861–7868.</p><pub-id pub-id-type="doi"/></element-citation></ref><ref id="B12" content-type="article"><label>12</label><element-citation publication-type="journal"><p>Kim M, Kim S, Lee J, et al., 2017, Serum Interleukin-33 as a Novel Biomarker for Acute Pancreatitis: A Preliminary Study, Journal of Clinical Gastroenterology, 51(6): e72–e77.</p><pub-id pub-id-type="doi"/></element-citation></ref></ref-list></back></article>
